Overview
To describe where something is in the body without confusion, clinicians agree on a single frame of reference. Every directional term is stated as if the person were standing in the anatomical position: standing erect, facing forward, feet slightly apart, arms at the sides, with the palms and toes pointing forward. Terms like “right” and “left” always refer to the patient’s own right and left, not the observer’s. Using this shared standard means a note written in one hospital is understood in any other.
Body Planes
An imaginary flat surface slicing through the body is a plane. Three planes are used constantly in anatomy and imaging (CT and MRI scans are read in these planes).

Directional Terms
Directional terms come in opposite pairs. Learning them as pairs makes them easier to remember.

Examples in use:
- The heart is superior to the stomach.
- The sternum (breastbone) is anterior to the spine.
- The thumb is lateral to the little finger in anatomical position.
- The knee is proximal to the ankle but distal to the hip.
- The skin is superficial to the muscles beneath it.
A few more pairs round out the everyday vocabulary. Bilateral means affecting both sides (bilateral leg swelling), while unilateral means one side only. Ipsilateral refers to the same side of the body, and contralateral to the opposite side. Notice that these terms are relative: the elbow is proximal compared to the wrist but distal compared to the shoulder, so you must always state the two structures being compared.
Body Cavities
Internal organs are housed in spaces called cavities, which support and protect them. The two main cavities each have subdivisions.
- Dorsal (posterior) cavity
- Cranial cavity — encloses the brain
- Vertebral (spinal) cavity — encloses the spinal cord
- Ventral (anterior) cavity
- Thoracic cavity — contains the heart (in the mediastinum) and the lungs (in the pleural cavities); separated from the abdomen by the diaphragm
- Abdominopelvic cavity — the abdominal portion (stomach, liver, intestines, spleen, kidneys) and the pelvic portion (bladder, reproductive organs, rectum)
Mapping the Abdomen
Because the abdomen holds so many organs, clinicians divide its surface to describe the location of pain, wounds, or masses. Two systems are common.
The Four Quadrants
A vertical and a horizontal line cross at the navel, creating four quadrants:

Quadrants are quick and are used constantly in emergency care. For example, appendicitis classically causes RLQ pain, gallbladder disease often produces RUQ tenderness, and stomach or spleen problems tend to localize to the LUQ.
The Nine Regions
A more detailed grid divides the abdomen into nine regions:
- Top row: right hypochondriac, epigastric, left hypochondriac
- Middle row: right lumbar, umbilical, left lumbar
- Bottom row: right iliac (inguinal), hypogastric (pubic), left iliac (inguinal)
Clinical Relevance
Precise location language guides real decisions. An EMT reporting “penetrating trauma to the LUQ” alerts the trauma team to possible spleen injury; a nurse charting “distal pulses present” confirms blood flow past an injury; a radiologist describes a lesion by plane and region so a surgeon can find it. Consistent use of anatomical position, planes, directions, and abdominal maps removes ambiguity from every hand-off, note, and order in patient care.
